By submitting this payment, I certify that I am authorized to use the payment method provided and authorize Carrier Credit Services, Inc. to charge the amount shown above. I acknowledge that this payment is being made to satisfy a valid outstanding obligation, that the amount and invoice(s) being paid have been reviewed and are accurate, and that I have had the opportunity to raise any billing questions or disputes prior to payment.

I agree that, once submitted, this payment is final and non-refundable except as required by law or expressly provided in CCS's Terms and Conditions / Refund Policy. I further agree not to initiate a chargeback or payment dispute with my card issuer or bank for any reason other than unauthorized use of my payment method, and I acknowledge that doing so may result in collection of the disputed amount, related fees, and reinstatement of the underlying obligation.
